Services

Case Management

Your case manager will meet with you and members of your care team to collaborate on your needs and design the best plan of care for you and your family.  A comprehensive home care plan will be established and followed by all disciplines.  All necessary supplies, medications and equipment will be ordered and available upon initiation of services.

You will have regularly scheduled meetings with your case manager to discuss the progression of your health services, evaluate your plan of care and assist with all other needs.  You may call or page the case manager with any emergent needs if they arise.  The case manager will arrange, provide and communicate your care to the other team members.  She will also attend all necessary medical appointments and provide updates to your physician as needed.
